Step 1: Register with PSO at BORN Ontario as a Quality Advisor/Medical Director 

Register with PSO at BORN Ontario by filling out and sending in a “quality advisor/medical director” registration form. Complete all of the required fields. You will receive a confirmation email to the address you provided on the form after you submit your request.

Quality Advisor/Medical Director Registration Form

Fill out this registration form if you are a quality advisor or a medical director who does not perform NT measurements, but oversees sonographers who do, and would like to receive communications regarding Ontario's NTQA program.

If you are a physician who has never performed, and would like to perform NT measurements and submit them to the Ontario MMS labs for first trimester screening, please register as a “new sonographer.”

If you are a FMF NT certified physician who has an Ontario NT ID number and has been performing NT measurements and submitting them to the Ontario MMS labs for first trimester screening, please register as an “existing sonographer.”

Tips for successfully completing your registration form

• Please enter an email address that you check regularly and can access from anywhere.
• Please enter a phone number where we can contact you directly (including a direct extension if applicable).
• Please enter your primary place of employment under the organization heading (i.e., the institution where you work the most hours).
• Please choose the prenatal screening lab where your primary place of employment or place of practice sends their prenatal screening samples.

Step 2: Log in to the BORN Information System (BIS) and review/update your personal contact details 

Please allow up to four weeks for the Ontario MMS labs to process your registration request. Once processed, you will get two emails to the email address that you included on your “quality advisor/medical director” registration form. One will contain your BORN login and one will contain your BORN password. Please check your junk mail folder because sometimes these emails filter into this folder.

You are now able to log in to the BIS, update your contact details in our system, and receive communications sent out by PSO at BORN Ontario about Ontario's NTQA program.
