Join BRIDGE-T1D Knowledge Exchange Events

BORN is pleased to share information about two virtual Knowledge Exchange events being hosted by the BRIDGE-T1D project team in July. Details about the project and the Knowledge Exchange Events are included below:

About the Project:

BRIDGE-T1D (Building Readiness & Independence During the Guided, youth-Engaged Transition to Adult Type 1 Diabetes Care) focuses on improving the transition from paediatric to adult care for youth living with Type 1 diabetes (T1D).  Using community-based participatory research and human-centred design approaches, the first phase of the study will co-design an audit and feedback based, youth-engaged Community of Practice (CoP) that brings together youth with lived experience, caregivers, healthcare providers, and researchers.  The goal is to strengthen transition readiness by integrating youth priorities into transition practices while supporting providers through share-T1Dd learning, audit and feedback, and peer discussion.  The study is led by Dr. Rayzel Shulman (SickKids) and Dr. Ian Zenlea (Institute for Better Health, Trillium Health Partners).

About the Knowledge Exchange Events:

The virtual events will be 2 hours in duration and will include:
  • A presentation on the project objectives
  • An overview of Communities of Practice
  • A panel discussion featuring youth living with T1D
  • A “Future Workshop” activity where participants will identify challenges, imagine ideal solutions, and develop realistic next steps
By the end of the event, the team aims to identify youth priorities for transition care activities that will help inform the transition model being developed.  No prior experience is necessary, and all participants will receive an honorarium of $35/hour.  Individuals interested in learning more or participating in the Knowledge Exchange Events are encouraged to complete this Expression of Interest form or contact the project team directly at hamna.khan@thp.ca
