BORN Webinar: Sociodemographic and Social Determinants of Health

This webinar introduces a survey designed to understand how sociodemographic (SD) and social determinants of health (SDH) data are collected and used across Ontario. It features care partner guests who share their journey with SD and SDH data collection and how this data is used to improve the delivery of care in their communities for equity-deserving groups.
