Membership Maternal-Child Screening Committee

Dear Department and Division Heads:

The Provincial Council for Maternal and Child Health (PCMCH) and the Better Outcomes Registry and Network (BORN Ontario) are seeking one member for the Maternal-Child Screening Committee.  The attached letter is sent to you for distribution to those individuals in your organization who may wish to apply for membership on the committee.  Attached is the Expression of Interest form which should be completed by applicants and then forwarded to us by fax or e-mail (details included on the form).  The deadline for sending the completed form is Monday, November 14, 2011.  Your assistance in distributing broadly throughout your organization and forwarding to other groups who may be interested, will be much appreciated.
