Expression of Interest PS Subcommittee

Dear Department and Division Heads:

The Provincial Council for Maternal and Child Health (PCMCH) and the Better Outcomes Registry and Network (BORN Ontario) are seeking two members for the Prenatal Screening Subcommittee of the Maternal-Child Screening Committee.  The attached letter is sent to you for distribution to those individuals in your organization who may wish to apply for membership on the subcommittee.  Attached is the Expressions of Interest form which should be completed by applicants and then forwarded to us by fax or e-mail (details included on the form).  The deadline for sending the completed form is Friday, October 7, 2011.  Your assistance in distributing broadly throughout your organization will be much appreciated.
