All requests made to BORN Ontario for access to health data will be managed in accordance with the provisions of the Ontario Personal Health Information Protection Act 2004 (PHIPA). Requests will be considered for purposes of research including quality assurance projects. Data may include personal health information and de-identified health information from databases for which BORN Ontario has custody.
Prior to completing the request form, consider emailing the BORN Ontario Scientific Manager (Science@BORNOntario.ca) to discuss your project and learn about: the timelines for data requests; the data request assessments conducted by the Electronic Health Information Laboratory (eHIL) at the Children’s Hospital of Eastern Ontario which determine the risk of re-identification; and the Research Ethics Board requirements.
The application process varies depending on the type of information you are requesting.
